I found this article in the November 25, 1969 issue of the Milwaukee Journal. Imagine if this continued to be the official policy of NASA? It's evident that, by the large number of reproductions from that era, the policy was changed. By the same token, it could also explain the rarity of Apollo 11 and 12 crew-worn patches and contractor (Grumman) issues. 

 

Apollo 11 - 4" - "Grumman" issue

This Apollo 11 patch is associated with Grumman as this version was most commonly found in collections by Grumman employees. Actual manuafturer unknown. 4"

Harder to find than Lion Brothers but appear a few times a month on eBay.

Apollo Lunar Module 1

Patch of unknown origin, commemorates Grumman's first lunar module, designated Apollo 5. Apollo 5 Launch vehicle: Saturn 1B Launch: 22 January 1968 Re-entry: 22 January 1968 (LM ascent stage) 12 February 1968 (LM descent stage) This was the first flight test of the Apollo LM. The flight verified ascent-and descent-stage propulsion systems, including restart and throttle operations. It also evaluated LM staging and fourth-stage orbital performance.
